First Aid Record

Each time a worker sustains a work-related injury, exposure, or illness, a first aid record must be completed. On a Polygon Construction Site, this form is the preferred method to capture the information described in OHS Guideline G3.19.

Note:

It is important to establish report sequencing to properly track the initial report and any subsequent reports created during follow-ups with an injured worker. 
Each completed record must be kept at the workplace for 3 years. All first aid records must be kept confidential.
