Permit - Ground Disturbance (Excavation)

While this permit is not generally required for intial bulk excavation*, once PCML is the prime contractor and as soon as anything has put underground as part of the overall project, temporary or otherwise, this permit must be used.

• a permit is issued for a specific task or area (e.g. trench drain along north side of building) and remains in effect until that task is complete
• open permits should be kept in the Site/Safety trailer for reference by the Site Team (not with the Trade doing the work) and filed once the task is completed
• the CSO or another member of the Site Team can sign off in the Work Completed: section

* general Polygon practice is that the Excavation or Civil On-site contractor is the initial prime contractor for the workplace.
