Sign out sheet - Fall Protection Equipment

This worksheet is for recording when the PCML Inventory of Fall Protection Equipment is signed out for use, and when it is returned.

Workers will need to indicate that they have received the equipment identified in good condition, have been previously trained in its use, and have conducted a pre-use inspection.
